Award Descriptions and Qualifications:
"The Ken King Community Service" Award: Recognizes an individual who has provided exemplary work and service in Whatcom County to create positive, local change in the care of those affected by dementia.
"Someone Who Cares" Award: Recognizes a care partner who has dedicated their life to show up for others day after day. This person displays kindness, patience, and empathy, providing compassionate care for others and ensuring no one walks their dementia journey alone.
"The Frank & Josselyn Winslow Advocacy" Award: Recognizes an individual who works to advance dementia awareness, support, and acceptance. This includes, but is not limited to, improving our systems and public policies, expanding access to dementia education and resources, and challenging the stigmas and societal perceptions related to dementia.
How to nominate someone: Send this form and include an explanation (100-700) words as to why you believe this person should receive this award. You may include the persons lived experiences/story, certifications, etc. Nominations under our word limit will not be considered for the award.
Due Date: Dementia Support Northwest must receive your nomination no later than September 1st, 2026, to be eligible. Winners will be recognized at the DSNW Fall Conference on October 15th, 2026, at the Silver Reef Event Center.
